Checking in and Health Form for Veterinary Campers

Completing the Application Process:

Campers who have been accepted to the N.C. Zoo's Veterinary Science Camp need to download and submit a Health Form to complete the application process. Health Forms may be FAXed (336-879-3963)  or mailed to the Zoo Society (4403 Zoo Parkway; Asheboro, NC 27205; attention: Jayne Owen Parker, Pd.D.)  Download your Health Form here.

Campers and parents should download this information sheet for directions on checking into the camp and communicating with staff.

If you submitted a credit card number to pay for your camper's deposit and you want us to charge the full

We have small number of partial scholarships for qualified campers.
Children from families enrolled in North Carolina’s Work First Family Assistance Program

Children in foster care

Children who have a parent serving in Iraq or Afghanistan

Children who have a parent who has already served in Iraq or Afghanistan

Children from families receiving emergency assistance from the Department of Social Services

Children from families with household incomes below $20,000 a year.
To apply for a scholarship, download this application form and return it with the requested documentation to N.C. Zoo Society; 4403 Zoo Parkway; Asheboro, NC 27205   Attention: Jayne Owen Parker, Ph.D.

Depending on campers' situations, scholarships will cover 50 to 97 percent of camp tuition.
